Registration

• For homeowners: Copy of deed to house/condominium or recent mortgage statement or recent property tax statement (no more than 45 days old)
• For renters:
  • Copy of updated residential lease, executed within 6-10 months of registration
  • If you do not have a lease then provide the Notarized Landlord Affidavit - (Click to view form)
• For individuals sharing housing - Notarized Living Arrangement Affidavit - (Click to view form)
  • In addition, please provide 3 proofs of residency from OWNER or LEASEE (1) Primary and (2) Supplementary
• Third-party statement relating to physical presence in the District of parent(s}/guardian(s) such as from a social services caseworker or other individual with knowledge of the situation

• ** 1 Must be a photo ID - Driver's License with current Croton address
• AND ** 1 of the following (listed below):
  • Utility or Other Bill(s) (No more than 45 days old):
    • Utility Bill (Con Ed, NYSEG)
    • Telephone (NOT Cell Phone}
  • Pay stub (No more than 45 days old)
  • Income tax form (From previous tax year only)
  • Membership documents based upon residency, for example library cards (No more than 45 days old}
  • Voter registration documents
  • State or other government-issued identification
  • Documents issued by federal, state or local agencies, for example, local social service agency or Office of Refugee Resettlement (No more than 45 days old)
  • Evidence of custody of the student, including, but not limited to, judicial custody orders or guardianship papers

• Certified transcript of a Birth Certificate or Baptismal Record
• Only if the above is not available, a Passport, Including a foreign passport
• Only if both of the above are not available, other documentation in existence for two or more years, including but not limited to: (Click to see options)
  • Official Driver's license
  • State or other government issued identification
  • School photo identification with date of birth
  • Consulate identification card
  • Hospital or health records
  • Military dependent identification card
  • Documents issued by Federal, State or local agencies (for example, local social service agency, Office of Refugee Resettlement)
  • Court orders or other court-issued documents
  • Native American tribal document
  • Records from non-profit international aid agencies and voluntary agencies

In order to provide your child with the best possible education, we need to determine how well he or she understands, speaks, reads and writes in English, as well as prior school and personal history.

• Proof of Parental Relationship/Guardianship and/or Proof that child resides with Parent(s)/Guardian(s):
  • Court Order or Agreement Regarding Guardianship/Custody, if available and parent(s) are willing to supply such documentation
  • Other evidence of guardianship of the student, including affidavit stating either:
    • that they are the parent(s) with whom the child lawfully resides OR
    • that they are the person(s) in parental relation to the child, over whom they have total and permanent custody and control, and describing how they obtained total and permanent custody and control
    • The District may also accept other proof, such as documentation that the child resides with a sponsor with whom the child has been placed by a Federal agency

• If your child received services through the Croton-Harmon CPSE, you DO NOT need to provide any Special Ed plan documentation at the time of registration.
• If your child received special education services (CPSE, CSE, or 504 accommodations) through ANOTHER school district:
